      Insurance

We accept the majority of major health insurances. The only way to be 100% sure if we take your specific insurance or verify coverage and/or reimbursement is to contact the customer service department on your insurance card. They have representatives to help with any insurance question.

Effective January 1st, 2025 we will only take United HealthCare Advantage plans for New Patients that qualify for Medicare.

For our qualifying Medicare establish patients, we will only be taking Traditional Medicare and all United Health Care Advantage plans.

For the 2025 open enrollment you will need to pick Traditional Medicare or United HealthCare Advantage plan to stay with our practice. 

​

For all commercial plans, contact the customer service department listed on your insurance to verify if we are part of your specific plan.

 We are still taking most commercial plans but want you to verify 100% with the plan to make sure you do not have any problems with your coverage.

IF THERE IS A PROVIDER/PCP NAME ON THE CARD OR YOU HAVE A HMO PLAN ONE OF OUR PROVIDERS MUST BE LISTED FOR YOUR INSURANCE TO PAY FOR YOUR CLAIM.

We will no longer be taking the following insurances:

  • MEDICAID

  • MEDICARE- NEW PATIENTS

  • CHP PLUS

  • ACCESS HEALTH COLORADO (COLORADO ACCESS)

  • COLORADO CHOICE HEALTH PLANS

  • HUMANA MEDICARE AFTER 1/1/2025

  • AETNA MEDICARE AFTER 1/1/2025

  • BCBS MEDICARE AFTER 1/1/2025

  • CIGNA MEDICARE AFTER 1/1/2025

We are partnering with you in your health care. Therefore, every patient needs an in-depth knowledge of their own insurance information. While we wish to provide the best care, any non-covered benefits are patient responsibility. Therefore, we motivate our patients to be very aware of what their personal health coverage plan contains.

Payments

Please send payments to Smoky Hill Family Medicine, call us or pay your bill online using our secure patient portal

 

If you have an outstanding balance, you must make a payment before scheduling another appointment.
